The Oxford County Community Health Centre (OCCHC) exists to address the needs of community members who face a range of barriers in everyday life. Our Housing Stability Team is growing!
We are seeking:
HOUSING STABILITY COMMUNITY CASE MANAGER – Ingersoll & Tillsonburg
MARCH 2025 - MARCH 2026 - CONTRACT
The Community Case Manager will be responsible for helping community clients with housing seeking, eviction prevention, landlord mediation, system navigation, etc. in Tillsonburg and Ingersoll.
The successful candidate will:
- Support individuals who are facing eviction-related issues, and prepare individuals for Landlord Tennant Board Hearings where necessary.
- Provide intensive case management including regular, continuous contact with client caseload.
- Collaborate with other system elements, accompany clients to appointments, provide proactive crisis planning, liaise with Oxford County Human Services in provision of Ontario Works, Ontario Disability Support Program, housing selection, and other basic needs programs and services.
- Connect the client with appropriate services or supports based on their needs, including e.g. physical healthcare, mental healthcare, addiction services, trauma services, vocational assessment and training, employment.
Compensation: Competitive compensation and in-lieu % benefits.
Hours of Work: 37.5 hours weekly (Monday – Friday).
Requirements:
- Post-secondary education in social services or related field
- A current Member (or eligible for membership) to a regulatory college, such as the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Considerable experience addressing the needs of individuals and families experiencing homelessness, or those at risk of homelessness
- Understanding of community agencies and a wide cross-section of service providers – preference will be given to candidates with demonstrated knowledge of Tillsonburg, Ingersoll & Area communities.

Education
